Purpose: To recognize an individual new practitioner who has demonstrated sustained contributions to WVSHP, SSHP, and/or the practice of pharmacy in WV through leadership, innovations, and commitment to the profession of pharmacy.

Eligibility:

• Licensed pharmacists in their first five (5) years of practice post-graduation
• Includes pharmacy residents practicing in the state

Award Components:

• Plaque or other display of recognition
• Complimentary 1 year WVSHP membership

Selection Criteria:

• Notable contributions to practice and/or the profession
• Demonstrated leadership, innovation, and commitment to pharmacy practice

Application Requirements:

• Self or peer nomination; if self, include brief personal statement
• CV
• 2 letters of support (if a peer nomination, submitter can count as one letter) (be prepared to provide name and email address for letter writers at time of submission)
