Purpose: To recognize individual pharmacist who has demonstrated sustained contributions to WVSHP, SSHP, and/or the profession of pharmacy in WV through pharmacy practice, patient care, education, advocacy, service, and leadership.

Eligibility:

• Open to all pharmacist members

Award Components:

• Plaque or other display of recognition
• Complimentary registration to annual meeting or 1 year membership

Selection Criteria:

• Significant and sustained contributions to pharmacy practice, patient care, education, and/or advocacy
• Leadership within the profession or community

Application Requirements:

• Self or peer nomination; if self, include brief personal statement
• CV
• 2 letters of recommendation (be prepared to provide name and email address for letter writers at time of submission)
